To enroll your child(ren) into our peer-support groups, please fill out the registration form using the button at the bottom of the page.

Once you submit the form, you will receive a call within 1-2 business days from our Director of Family Services to schedule a family orientation. This orientation is a check-in to deliver information to you and your family about the program before getting started. This is when you will receive a tour of our facility and be placed in a program night schedule for our peer-support groups.

After your orientation, you will receive specific information about your start date.

If you prefer a hard copy of the application, you can download a PDF version of our Family Intake Questionnaire. You can either drop off the completed application at our office Monday-Friday from 9:00 A.M. to 5:00 P.M. or mail the application to:
