REGISTER FOR ERIN'S HOUSE
To enroll your child(ren) into our peer-support groups, please fill out the registration form using the button at the bottom of the page.

Once you submit the form, you will receive a call within 3-4 business days from our Director of Family Services to schedule a family orientation. This orientation is a check-in to deliver information to you and your family about the program before getting started.

After your orientation, you will receive specific information about your start date.

If you prefer a hard copy of the application, you can download a PDF version of our Family Intake Questionnaire. You can either drop off the completed application at our office Monday-Friday from 9:00 A.M. to 5:00 P.M. or mail the application to:

Please note: Our office is closed until January, and responses may take a little longer. Please know your application will be received and a staff member will be in contact when we return.
While we are away if your family needs immediate phone support, please call the office at 260-423-2466 EXT 101 and someone will be in contact with you within 1-2 days.
